An opportunity has arisen for a community staff nurse to join the Marlow Community Healthcare Team. The post is to provide nursing interventions to patients in their home environment, avoiding hospital admissions and facilitating discharges from the acute. The post will involve working with healthcare assistants and multidisciplinary working with clinicians in the ACHT.
You should have a wide range of clinical expertise within the NHS as well as experience working in a multidisciplinary team environment. The post holder must be able to demonstrate their professional skills and competence in medicines management, care and treatment delivery. You must be innovative in your approach and willing to work with other members of the community team and ward staff to ensure a high quality service delivery. You must be willing to develop your own knowledge and skills.
In order to respond to the healthcare needs of the local population, the ACHT's now work shift patterns across a 24 hour period, 7 days a week; ensuring the delivery of a service which can react to the patients needs as and when required. The successful candidates will be expected to work within these shift patterns.
