The teams are made up of Physiotherapists, Speech & Language Therapists, Dietitians, Nurses, Occupational Therapists, Rehabilitation Technicians, Doctors, Age Connects brokers and Social Services staff. They work closely with the local acute hospital teams and GPs to provide timely discharge of patients from hospital and support at home to reduce unnecessary hospital admissions where this is appropriate.

The ability to speak Welsh is desirable for this post.
