Our vacancy
12 month fixed term covering maternity leave The Community Nursing service aims to prevent hospital admissions and facilitate hospital discharge to allow people to remain in their own homes. The District Nursing Service provides nursing care to housebound patients in non-hospital settings such as a patient's home and residential homes, whilst delivering high quality comprehensive holistic nursing care. The community nurses provide health education and promotion advice, they also provide palliative care, End of life care, wound management, catheter and continence care. The teams work closely with other health and social care providers to support people in their own homes. The post holder will be based in Midsomer Norton alongside other health and social care teams covering identified GP practice clusters. Most of the time will be spent working in the patient's own homes where environments may be difficult and pose manageable risks or unavoidable hazards. A risk assessment of the home environment will always be completed.
As a Community Staff Nurse you will deal with a variety of patients and conditions and contribute to the overall performance of the team by being a proactive, enthusiastic team member. The role will also involve: Assess, plan, evaluate and deliver individualised patient care. Undertake terminal care inclusive of setting up syringe drivers for symptom management/pain and changing doses of drugs, including controlled drugs, within the parameters as prescribed by GP according to the patient's needs. To have knowledge of the drugs used in symptom control management. Complete assessments of clients using observation, measurement, verbal and written documentation, including medical history, and environmental factors, without direct supervision; including complex care assessments in consultation with senior grade. Advise and recommend treatment, liaise with other Health Care Professionals accepting referrals written and verbal. Participate in clinical audits and data collection Contribute to service development and link nurse role Manage the District Nurse Caseload in the absence of the Team Leader within agreed boundaries
The essential qualities we are looking for in our Community Staff Nurse applicants are:
* Holds recognised qualifications within the professional area at e.g. Degree/ BTEC Advanced Professional Diploma/ Graduate certificate level/NVQ 4/VQ4 or equivalent. Must hold a degree or qualification recognised as equivalent by professional body if an essential requirement of the post and with the appropriate registration
* Have significant and relevant practitioner experience in a similar sized health/ social care organisation and be competent in a wide range of relevant clinical skills.
* Knowledge and understanding of the legislation that applies to care and health work.
* Good working knowledge of typical work related equipment, systems, procedures and technology.
* Good Communication skills including IT literacy and knowledge of computer systems
* Contribute to the development of others, teaching and assessing and mentoring of student nurses.
* Comprehensive knowledge of standardised rules, procedures within own area and across disciplines relevant to the job role.
* Experience of interpreting and explaining routine and non- routine technical information.
* Evidence of ability to resolve routine and non- routine problems using sound logic.
* Evidence of meeting regular targets typically of short term duration
