As part of the Vanguard project, which involves integration of a range of community providers, we are looking for a band 6 nurse to support people going home from hospital and reduce hospital admissions. The priority for this role is to keep people happy and healthy at home.

An exciting opportunity has arisen for you to develop your clinical skills by working in this integrated community outreach team.
ERS@H is a multi-disciplinary team who provide up to six weeks of assessment and treatment for patients within their own home,

The post is looking for an experienced nurse to work with in our Community Team, working across sites, in FPH and also Fleet. The post holder will assist clinically with patients in their home, but also as part of the assessment team for ERS@H, within FPH. The patients will have a wide range of diagnoses, therefore varied clinical experience plus previous work in the community would be an advantage. Training and competencies will be provided.

The post holder also supports the Lead Nurse in development and co-ordination of the service, training of Allied health professionals in nursing skills and on a daily basis line-manages the Rehabilitation Support Workers.

The successful applicants must be able to independently travel to and from a patient's home. The service runs seven days a week and so the post holder must be available to work at different times according to their shift pattern.
